Chronic obstructive pulmonary disease (COPD) is a global health problem that has a significant impact on public, economic, and social health. One of the main risk factors that causes irreparable lung damage is smoking. Therefore, early detection of COPD is very important to prevent increased morbidity and mortality rates, especially in active smokers. Screening and early treatment are very important in reducing the development and impact of COPD. In this study, we conducted an observational study with a latitude cut design in the smoker population to compare the effectiveness of two COPD screening questionnaires, namely PUMA and Capture, in identifying undiagnosed respiratory disease and exacerbation risk.

In this study, data was collected through interviews using two COPD screening questionnaires, namely PUMA and Capture, which was then followed by a Spirometry examination. Bivariate analysis using the Spearman test is used to assess the relationship between capture and PUMA scores based on the results of Spirometry. ROC analysis is used as a sensitivity test and specifications to determine the threshold value in the capture and puma questionnaire in identifying COPD.

The results showed that the characteristics of smokers were in the age range of 51-60 years (38.1%) with a smoking history begins at the age of 15-20 years (65.8%). The majority of smokers are male (81.5%), with the intensity of smoking being the most common (44.8%). Spirometry results show that most smokers experience obstruction (65.1%), especially severe obstruction (42.5%). The optimal threshold value for PUMA score is ≥ 6, with a sensitivity of 72.55% and specifications of 84.00%. As for capture, the threshold value found is 4, with a sensitivity of 70.83% and the specifications of 64.29%.

From the results of the analysis, it can be concluded that the PUMA questionnaire has a relatively higher sensitivity and specifications compared to the capture questionnaire. This shows that the effectiveness of the PUMA questionnaire in detecting COPD is better than capture. The PUMA questionnaire has a higher sensitivity and specificity, which means that it is more accurate in identifying individuals with COPD. This is an important finding, as early detection of COPD is crucial in preventing the progression of the disease and reducing the burden caused by COPD.

This study provides an important insight into the importance of using the right screening tool for early detection of COPD in smokers, in order to reduce the impact of this disease which has become a public health problem.
