Profile of Child Patient Visits at the H. Adam Malik Hospital and North Sumatra Hospital in 2020

In the context of health services in Indonesia, especially in the pandemic era, information about the visit of child patients at the Emergency Room (IGD) is still limited. This study focuses on the analysis of the profile of pediatric patients treated at the Adam Malik Hajj Hospital (HAM) and the University of North Sumatra (USU) Medan in 2020. By utilizing medical record data from the hospital information system, this study is expected to provide a clearer picture of the characteristics and condition of pediatric patients visiting the emergency room of the two hospitals.

Research Background

In 2020, the COVID-19 pandemic has changed many aspects of health services, including patient management in hospitals. Limited data about pediatric patients in the emergency room makes this research more relevant. The purpose of this study was to analyze and understand the profile of pediatric patients treated at the Adam Malik Haji Hospital IGD and North Sumatra University Hospital.

The COVID-19 pandemic has put a significant strain on the healthcare system, leading to a shortage of medical resources and a surge in patient admissions. In Indonesia, the pandemic has also highlighted the need for more effective patient management strategies, particularly in pediatric care. The emergency room is a critical area of the hospital where patients with urgent medical needs are treated. However, limited data on pediatric patients in the emergency room makes it challenging to develop effective patient management strategies.

Methodology

This study uses a retrospective descriptive method that collects data from the medical record of patients visiting the emergency room from January to December 2020. Adam Malik Haji Hospital is a type A hospital that functions as a reference, while the University of North Sumatra Hospital is a type C hospital.

Research Result

During this period, there were 3,462 patients visiting the emergency room, with the majority of the age of patients in the range of 11-18 years. Data shows that USU Hospital has 267 patients and HAM hospitals have 971 patients in the age group. Of the total patients, there are 428 (56%) male and 336 (44%) of women from USU Hospital, and 1,574 (58%) male and 1,124 (42%) of women from HAM Hospital.

The study found that the majority of patients visiting the emergency room were adolescents, with more male proportions than women. The data also showed that the majority of patients were from HAM Hospital, with 971 patients in the age group of 11-18 years. In contrast, USU Hospital had 267 patients in the same age group.

Diagnosis and Comorbidity

In terms of diagnosis, HAM Hospital recorded the highest number of trauma cases of 325 (12%), while USU Hospital reported that the fever was not specified with a total of 107 (14%). There are also 205 (7.6%) patients suspected of being exposed to Covid-19 at HAM Hospital and 57 (7.7%) at USU Hospital. In terms of treatment, 942 (34.5%) patients were treated and 1,684 (62.4%) patients were treated at HAM Hospital, while in USU Hospital, 435 (56.9%) Patients were treated and 321 (42%) inpatient.

The study found that the most common diagnosis was trauma and COVID-19 in HAM Hospital, while fever was not specified as the most common diagnosis in USU Hospital. The data also showed that a significant number of patients were suspected of being exposed to COVID-19, with 205 patients at HAM Hospital and 57 patients at USU Hospital.

Of the total visits, 2,539 (94.1%) patients in HAM Hospital and 666 (87.2%) in USU Hospital do not have a comorbid. Meanwhile, 159 (5.9%) patients in HAM Hospital and 98 (12.8%) patients in USU Hospital have a comorbidity.

The study found that the majority of patients did not have a comorbidity, with 94.1% of patients in HAM Hospital and 87.2% of patients in USU Hospital not having a comorbidity. However, a significant number of patients had a comorbidity, with 5.9% of patients in HAM Hospital and 12.8% of patients in USU Hospital having a comorbidity.

Conclusion

This study shows that the majority of patients visiting the emergency room are adolescents, with more male proportions than women. The most common diagnosis is trauma and covid-19 in HAM Hospital, while fever is not specified into the most diagnosis in USU Hospital. This data provides an important picture for the management of child patients in the emergency room, as well as being the basis for improvement of future health services. In the future, more comprehensive data collection is needed to support decision making in health policies.
