Rationality of the Use of Intravenous Preparations in Diarrhea Sufferers in Tk Hospital. II Putri Hijau Kesdam I Bukit Barisan

Introduction

Diarrhea is a serious health problem in developing countries, including Indonesia, with a high level of morbidity and mortality. In handling diarrhea, intravenous preparations are one of the most widely used drugs. However, the use of these preparations is not always rational, and this can lead to adverse effects and decreased treatment effectiveness. This study aims to evaluate the rationality of the use of intravenous preparations during diarrhea therapy, which includes appropriate aspects of the patient, precisely indicated, appropriate drugs, and exactly the dose.

Background

Diarrhea is a common disease that affects people of all ages, and it is a major public health problem in Indonesia. According to the World Health Organization (WHO), diarrhea is the second leading cause of death among children under the age of five, and it is also a significant cause of morbidity and mortality in adults. In Indonesia, diarrhea is a major cause of hospitalization, and it is estimated that over 1 million people are hospitalized due to diarrhea each year.

Intravenous preparations are widely used in the treatment of diarrhea, and they are often administered in hospitals and clinics. However, the use of these preparations is not always rational, and this can lead to adverse effects and decreased treatment effectiveness. In this study, we aim to evaluate the rationality of the use of intravenous preparations during diarrhea therapy, and to identify areas for improvement.

Methodology

This descriptive retrospective study was conducted at the Medical Record Installation of Tk. II Putri Hijau Kesdam I Bukit Barisan Medan. The criteria used are inpatients who experience diarrhea due to bacterial infections during the period January to December 2018. A total of 90 patients were included in the study, and their medical records were reviewed to evaluate the use of intravenous preparations.

Results

From the results of the study, we found that there were some inaccuracies in the use of drugs. Specifically, overdose occurred in Ceftriaxone in 12 cases and in ciprofloxacin in 9 cases. In addition, we also found adverse drug interactions, such as a combination of ringer lactate and ceftriaxone which caused incompatibility in 17 cases, as well as a combination of ciprofloxacin and ondansetron which could increase heart rhythm in 9 cases.

Discussion

The results of this study indicate that the use of intravenous preparations in diarrhea therapy is not always rational. Overdose and adverse drug interactions are common problems that can lead to adverse effects and decreased treatment effectiveness. These findings are consistent with previous studies that have reported similar problems with the use of intravenous preparations in diarrhea therapy.
