Introduction

Medical records play a crucial role in the hospital service system, as they contain a comprehensive record of every service and action provided to patients. However, the results of a preliminary survey conducted at the Medan Hajj Hospital in 2009 revealed that only 55% of the 100 medical record files taken from the inpatient room were completely filled by the nurse. This indicates a significant problem in the process of filling in medical records at the hospital. The low level of completeness of filling medical records by nurses is allegedly related to individual and organizational factors. Therefore, this study aims to investigate the influence of these two factors on nurse motivation in filling medical records.

Medical records are essential for providing high-quality healthcare services. They contain a comprehensive record of every service and action provided to patients, including diagnoses, treatments, and test results. However, the completeness and accuracy of medical records are often compromised due to various factors, including nurse motivation. Nurse motivation is a critical factor that affects the quality of medical records, as motivated nurses are more likely to complete medical records accurately and on time.

Methodology

This study employed the Explanatory Survey Method to investigate the influence of individual and organizational factors on nurse motivation in filling medical records. The study involved all nurses in the inpatient room of the Medan Hajj Hospital, totaling 149 people, who were used as samples. Data was collected through interviews using a questionnaire and analyzed using multiple logistics regression tests with a significance level of α = 0.05.

Results

The results of this study showed that individual factors, such as needs, goals, and ability, have a significant influence on nurses' motivation in completing medical record filling (P <0.05). However, the attitude variable did not show a significant effect (P> 0.05). This indicates that although the nurse's attitude towards filling in the medical record is important, other factors such as needs, goals, and ability play a more dominant role in motivating them.

Organizational factors, such as salary, occupational security, worker relations, supervision, and appreciation, also have a significant influence on the motivation of nurses in completing the filling of medical records (P <0.05). Interestingly, the salary variable has the most dominant influence on nurse motivation. That is, adequate and fair salary is an important factor that can increase nurses' motivation to be more serious in filling medical records.

Discussion

The results of this study highlight the importance of individual and organizational factors in influencing nurse motivation in filling medical records. The findings suggest that nurses' motivation is influenced by their needs, goals, and ability, as well as organizational factors such as salary, occupational security, worker relations, supervision, and appreciation. The results also show that the motivation of nurses in filling medical records was relatively low, which was 50.3%. This indicates the need for efforts to increase the motivation of nurses so that they are more encouraged to complete the filling of medical records accurately and on time.

Improved ability through training: Training that focuses on the procedures and completeness of filling medical records, with the method of lecture, discussion, and coaching can increase the knowledge and skills of nurses in filling medical records.
SOP confirmation and application of Permenkes: Medan Hajj Hospital Management needs to reiterate the Standard Operating Procedure (SOP) of filling medical records and ensuring its application consistently based on Permenkes Number 269/Menkes/PER/III/2008 concerning Medical Records.

In conclusion, this study highlights the importance of individual and organizational factors in influencing nurse motivation in filling medical records. The findings suggest that nurses' motivation is influenced by their needs, goals, and ability, as well as organizational factors such as salary, occupational security, worker relations, supervision, and appreciation. The results also show that the motivation of nurses in filling medical records was relatively low, which was 50.3%. Therefore, efforts should be made to increase the motivation of nurses so that they are more encouraged to complete the filling of medical records accurately and on time. By implementing the right strategy, the management of Medan Hajj Hospital can increase the motivation of nurses in filling in medical records so that the quality of medical records becomes better and the health service system in the hospital can increase significantly.
