Lung disease Chronic obstruction (COPD) is a serious health problem, especially in the acute exacerbation phase. The right treatment is very important to prevent further complications. However, the use of antibiotic therapy in acute exacerbation COPD patients can vary, and each therapeutic model can provide different clinical results. Therefore, analysis of cost effectiveness is needed to maximize the results of treatment while controlling the costs incurred. The use of antibiotics in COPD patients is a common practice, but the cost-effectiveness of these treatments is not well understood . This study aims to evaluate the effectiveness of the cost of using antibiotic therapy models in acute exacerbation COPD patients who are hospitalized in the Central General Hospital (RSUP) H. Adam Malik in Medan.
This study aims to evaluate the effectiveness of the cost of using antibiotic therapy models in acute exacerbation COPD patients who are hospitalized in the Central General Hospital (RSUP) H. Adam Malik in Medan. The primary objective of this study is to determine the most cost-effective antibiotic therapy model for acute exacerbation COPD patients . The study also aims to identify the characteristics of patients who are most likely to benefit from each antibiotic therapy model.
In this study, a descriptive cohort design was used, where data was obtained from the medical record of inpatients with a diagnosis of acute exacerbation COPD as many as 49 patients at H. Adam Malik Hospital in the January to December 2017 period. The study population consisted of patients who were hospitalized with a diagnosis of acute exacerbation COPD . The characteristics of patients were analyzed using descriptive statistical methods with program assistance SPSS version 21. To determine the most cost-effective antibiotic costs, the Cost Effectiveness Ratio (CERS) and Incremental Cost Effectiveness Ratio (ICER) methods are used. The outcome obtained is measured from the percentage of patients who have a normal number of leukocytes (WBC) on the 3rd day of treatment .
The results showed that most patients who experienced acute exacerbation COPD were male (81.6%) with an average age of 64 ± 1.5 years. The majority of patients were male, which is consistent with previous studies . The most widely given antibiotic therapy model to patients is ceptriaxone injection (44.9%), and this model shows the highest outcome with 81.8% of patients having normal WBC. The ceptriaxone injection model showed the highest outcome, which is consistent with previous studies . From the cost analysis, the Cerfriaxone antibiotic value was recorded at Rp 33,907.00, while the combination of sephriakson + azithromycin was Rp. 47,843.00 and the combination of sephriakson + siprofloxacin was Rp 141,879.00. The cost analysis showed that the Cerfriaxone antibiotic was the most cost-effective option . In addition, the ratio of the ICER value shows that sephriaksone has the lowest value (Rp. 140,291.00) compared to the combination of sephriakson + azithromycin.
Based on the results of the cost effectiveness analysis, the most effective antibiotic for the treatment of acute exacerbation COPD patients at H. Adam Malik Hospital is the injection of sephriaxone. The results of this study suggest that the injection of sephriaxone is the most cost-effective option for the treatment of acute exacerbation COPD patients . Therefore, sephriakson is recommended as a pharmacoeconomic standard in acute exacerbation COPD treatment, given the lower cost and clinical outcomes that are better compared to other antibiotic therapy models. This study shows the importance of evaluating costs in treatment to ensure that patients get therapy that is not only clinically effective but also efficient in terms of cost . With the application of this recommendation, it is expected to improve the quality of patient care and the efficiency of the use of resources in hospitals.
Based on the results of this study, the following recommendations are made:
The injection of sephriaxone should be used as the first-line treatment for acute exacerbation COPD patients .The cost-effectiveness of antibiotic therapy models should be evaluated regularly to ensure that patients receive the most cost-effective treatment .Further studies should be conducted to evaluate the cost-effectiveness of other antibiotic therapy models for acute exacerbation COPD patients .
