One such complication is hypertensive retinopathy, which causes damage to the retina or surrounding blood circulation due to high blood pressure. Hypertensive retinopathy can cause blindness if not detected and treated quickly. Generally, this condition detection is carried out through funduscopy examination by an ophthalmologist. However, the number of eye doctors in Indonesia is very limited and the distribution is uneven. In addition, funduscopy analysis that is currently carried out manually requires a long time and has the potential to cause errors, which can inhibit proper treatment for patients.

The Need for Automated Detection of Hypertensive Retinopathy

Therefore, we need a method that can help detect hypertensive retinopathy automatically through fundus images. In this study, the method used is the Deep Belief Network (DBN), which is one of the techniques in in-depth learning (deep learning). Retinal fundus image is used as input data in the image processing process. This process involves several important steps before the identification stage, including:

Pre-processing: Enhancing Image Quality

Pre-processing: This first step includes changing image size, green channel separation, and the application of CLAHE (Contrast Limited Adaptive Histogram Equalization). This technique aims to increase image contrast so that important features can be more easily identified.

Filtering: Reducing Noise and Interference

Filtering: The average filter is applied to reduce noise in the image, which can interfere with the next analysis process.

Background Expenditures: Focusing on the Retinal Area

Background Expenditures: Eliminating irrelevant background parts so that the focus of the analysis remains in the retinal area.

Segmentation: Extracting Important Features

Segmentation: Thresholding process segmentation is carried out to separate important objects in the image, which is then followed by extraction of features using GLCM (Gray Level Co-Ooccurren Matrix). GLCM is used to extract the texture information of the image which is an important indicator in diagnosing hypertensive retinopathy.

Data Collection and System Test Results

In this study, the data used came from several sources, namely the Stare Site (Structure Analysis of the Retina), Dr. Hagis, Kaggle.com, and Prima Vision Hospital and the University of North Sumatra Hospital. The total data used are 800 images, consisting of 640 data for training, 80 data for validation, and 80 data for testing.

System test results show excellent performance in identifying hypertensive retinopathy. Validation accuracy reaches 95%, test accuracy of 92.5%, sensitivity 90.4%, and specificity of 94.7%. These numbers indicate that the DBN method can be relied upon in detecting hypertensive retinopathy with a minimum error rate.

Conclusion and Future Directions

With this technological development, it is expected to increase access to the diagnosis of hypertensive retinopathy in Indonesia, especially in areas that lack specialist medical personnel. This automation method not only accelerates the diagnosis process, but also minimizes the possibility of errors, so that patients can get more appropriate and faster treatment. Along with technological advances, the integration between health and information technology will further facilitate the diagnosis and treatment of diseases that can endanger public health.
